LOGISTICS REGISTRATION FORM

[ ] I want campus housing. Enclosed is my check for $25 per person/per night for ___ Thurs night; ___ Fri night; ___ Sat night. No dorm housing can be arranged after July 5th.

My roommate will be ________________. [You will be housed in the newest dorm. Rooms are simple. You will be given towels/sheets and expected to make your own bed. You may want to bring a reading lamp; students take theirs home. Bathrooms are in the pod but not in your room, so bring a bathrobe. There will be several groups on campus. When dorms are full, no additional campus housing will be available. NONE. All rooms will be assigned first come/first served. Those who pay last may get non-airconditioned rooms; each year we have more and more requests for campus housing.] If there are four of you who want to be together, ask for a pod.]

[ ] I want to reserve/pay for ___ places for the banquet: $15 per person is enclosed. [NO BANQUET RESERVATIONS CAN BE ACCEPTED AFTER JULY 5th, and when we have reached 112, the room is full. You will find your tickets in the folder you pick up at registration.]

[ ] I am/we are vegetarians. [St Olaf accomodates vegetarians/vegans very well. Banquet will not be vegan. Both vegetarian lasagne and fish will be offered. Call to learn the rest of menu.]

[ ] I/we will want the meal plan. Two breakfasts/two lunches for $26.63 per person.

[ ] I/we will not participate in the meal plan, but we will help keep the cafeteria open by eating 4 meals [2 breakfasts/2 lunches] in the cafeteria.

[ ] I will want to buy ___ [indicate number] jugs of distilled water during the conference at $1 per gallon. [You may prefer to bring your own water for your dorm or hotel room. We provide room temperature distilled water/cups for drinking in the foyer of the recital hall. Water elsewhere on campus will not be distilled water.]
